1. GENERAL LABOR PROTECTION REQUIREMENTS

1.1. This Instruction provides for basic labor protection requirements for orthopedic traumatologists.
1.2. To work as a traumatologist-orthopedist, persons are allowed to be at least 18 years old who have a higher professional education in the specialty "General Medicine" or "Pediatrics", postgraduate professional education (internship and (or) residency) in the specialty "Traumatology and Orthopedics", a specialist certificate in the specialty "Traumatology and Orthopedics" without presenting requirements for work experience, as well as having no medical contraindications and passed:
- training on labor protection, testing of knowledge of labor protection requirements;
- training in electrical safety rules, testing knowledge of electrical safety rules in the scope of job duties;
- training in fire safety rules, testing knowledge of fire safety rules in the scope of job duties;
- training in safe methods of performing work and methods of providing first aid to the victim in case of accidents at work;
- checking knowledge of safe methods and techniques for performing work;
- introductory and initial briefing on labor protection at the workplace;
- internship at the workplace (for 3-14 shifts, depending on the length of service, experience and nature of work);
- preliminary and periodic medical examinations.
1.3. In the process of performing work, the orthopedic traumatologist may be affected by the following hazardous and harmful production factors:
- toxic effects of various chemicals that are part of medicines, etc., funds when absorbed through the mucous membranes and skin;
- risk of injury (damage to hands when using stabbing, cutting tools, glassware, etc.);
- sharp edges, burrs and uneven surfaces of medical equipment, tools and devices;
- danger of electric shock;
- electromagnetic radiation (MRI);
- ultrasound radiation (ultrasound);
- X-ray radiation;
- irrational organization of the workplace;
- insufficient illumination of the working area;

Sources of harmful and hazardous production factors:
- faulty equipment or improper operation;

1.4. The orthopedic traumatologist notifies his immediate supervisor about any situation that threatens the life and health of people, about every industrial accident, about the deterioration of his health, including the manifestation of signs of an acute illness.
1.5. A traumatologist-orthopedist must undergo training in labor protection in the form of: introductory briefing, initial briefing at the workplace, special training in labor protection in the scope of job duties upon admission to work during the first month, then - as necessary, but not less often once every three years.
1.6. The traumatologist-orthopedist is provided with sanitary clothes in accordance with the current regulations.
The issued sanitary clothing and other personal protective equipment must correspond to the nature and conditions of work, ensure labor safety, and have a certificate of conformity.
Personal protective equipment for which there is no technical documentation is not allowed for use.
1.7. Personal clothing and sanitary clothing should be stored separately in lockers and dressing rooms. It is prohibited to take sanitary clothes outside the medical organization.
1.8. Premises of a medical organization should be equipped with first-aid kits with a set of medicines, dressings and tools necessary for providing first aid in case of accidents.
1.9. An orthopedic traumatologist should know:
- rules for the provision of emergency medical care for fractures and other injuries of the musculoskeletal system;
- the procedure for organizing the workplace, taking into account the measures of personal, sanitary, technical and fire safety;
- device, principle of operation of the equipment used, instruments, apparatus;
- instructions for the operation of equipment, instruments and apparatus;

1.10. While on the territory of a medical organization, in production and household premises, at work areas and workplaces, an orthopedic traumatologist is obliged to:
- timely and accurately follow the rules of the internal labor regulations, orders of the administration, provided that he is trained in the rules for the safe performance of this work;
- comply with the requirements of local regulations on labor protection, fire safety, industrial sanitation, governing the procedure for organizing work at the facility;
- observe labor discipline, work and rest regime;
- take good care of the property of the employer;

1.11. Smoking and eating is allowed only in specially designated places. Wash hands thoroughly with soap and water before eating.
1.12. 2. LABOR PROTECTION REQUIREMENTS BEFORE STARTING WORK

2.1. Ensure personal safety during work:
2.1.1. Arrive at work in advance to avoid haste and, as a result, falls and injuries, while:
- do not go up or down the stairs;
- do not sit down or lean on fences and random objects;
- pay attention to safety signs, signals and comply with their requirements.
2.1.2. Check the availability and serviceability of personal protective equipment, put on them and sanitary clothing (suit (dressing gown), cap, slippers, mask), put everything in order.
Sanitary clothing should not have fluttering ends, sleeves and collars should be tied, hair tucked under a cap.
During the working day, clothes must be kept clean.
2.1.3. Do not pin the clothing with pins, do not keep sharp, fragile objects in clothing pockets.
Do not keep personal toilet items, cigarettes and other foreign objects in the pockets of a suit (dressing gown).
2.1.4. Wash your hands with soap and water. Fingernails should be trimmed short, unpainted and unadorned.

2.1.6. Prepare the workplace for safe work:
- inspect it, remove all unnecessary items;
- check the approaches to the workplace, evacuation routes for compliance with labor protection requirements;
- establish the sequence of operations;
- check the availability of fire-fighting equipment, first-aid kit (its complete set).
2.1.7. Check by visual inspection:
- no cracks and chips on the cases of sockets and switches, as well as the absence of bare contacts;
- Reliability of closure of all current-carrying and starting devices of medical equipment;
- the presence and reliability of grounding connections (absence of breaks, strength of contact between metal non-current-carrying parts of the equipment and the grounding wire);
- the integrity of the insulation of electrical wires and power cords of electrical appliances, the serviceability of safety devices;
- sufficiency of lighting of the workplace;
- absence of foreign objects around the equipment;
- condition of floors (absence of potholes, irregularities, etc.);
- the presence and accessibility of items that patients can injure staff and themselves.
2.1.8. Report all detected faults and malfunctions to the immediate supervisor (chief doctor, head of the department) and (or) the employee responsible for eliminating the identified violations (senior nurse, etc.).
2.2. Ensure safety and allow subordinates to work:
2.2.1. Visually check the general condition of the subordinate employee.

2.5. A traumatologist-orthopedist should not start work in the presence of the following violations of labor protection requirements:

- in the absence or malfunction of personal protective equipment;

- without passing a periodic medical examination.

3. REQUIREMENTS FOR LABOR PROTECTION DURING WORK

3.1. Do not allow untrained and unauthorized persons (including the sick) to work.
3.2. When performing medical procedures, use disposable surgical gloves. Once removed, gloves are not reused.
3.3. All injuries on the skin of the hands should be sealed with BF glue or adhesive plaster before putting on gloves.
3.4. During medical procedures, do not touch your eyes, nose, mouth, unprotected skin areas with your gloved hands.
3.5. During work, strictly observe the requirements of asepsis and antiseptics, the rules of personal hygiene. Demand the same from the personnel subordinate to him. Before and after each manipulation, the orthopedic traumatologist must wash his hands, followed by their treatment with one of the licensed bactericidal drugs.
3.6. After performing any procedures and removing gloves, wash your hands thoroughly with soap and warm running water for at least 10 seconds. Do not use hard brushes to wash your hands. When wiping hands, rubbing movements should be avoided, only an individual towel should be used.
3.7. Do not allow medications to come into contact with your hands.
3.8. Take special care when working with cutting and piercing objects (scalpels, scissors, glass objects). When performing medical procedures, it is necessary to take (take) the instrument only by the handle, constantly monitor the position of its sharp ends.
3.9. Monitor the integrity of glass parts of equipment and medical utensils, do not use damaged glass objects.

3.13. In case of detection of faulty medical equipment, apparatus, tools, other violations of labor protection requirements that cannot be eliminated on our own, and a threat to health, personal or collective safety, the orthopedic traumatologist should inform the management about this. Do not start work until the identified violations are eliminated.
3.14. Fulfill the sanitary and hygienic requirements for the ventilation regime, temperature regime.
3.15. Exercise control over the observance by subordinates of the rules of labor protection and safety measures, production and labor discipline.
3.16. Be attentive, not distracted by extraneous affairs and conversations, do not rush.
3.17. When working together, coordinate your actions with the actions of other employees.
3.18. Noticing a violation of labor protection requirements by another employee, the traumatologist-orthopedist should warn him about the need to comply with them.
3.19. Observe the rules for moving in the room and on the territory of the medical organization, use only the established passages.

3.22. When performing work, an orthopedic traumatologist is FORBIDDEN:

- leave medicines, medical devices, equipment unattended;
- store and use drugs without labels, as well as in damaged packaging;
- transfer medicines from one package to another, replace labels;
- store medicinal products together with food products, solutions for technical purposes (handling hands, tools, furniture, linen, etc.), etc.;
- to perform self-opening and repair of equipment, instruments and apparatus. The repair must be carried out by a specialist;
- use faulty equipment, as well as devices and equipment, the handling of which he is not trained;
- perform work without using the necessary PPE;

4. REQUIREMENTS FOR LABOR PROTECTION IN EMERGENCY SITUATIONS

4.1. When eliminating an emergency, it is necessary to act in accordance with the approved emergency response plan.
4.2. If equipment, devices and apparatus malfunctions are detected, as well as other conditions that threaten the life and health of workers, the orthopedic traumatologist should stop working and report them to the immediate supervisor (chief doctor, head of the department) and the employee responsible for the production control.
4.3. When a fire source appears, it is necessary:
- turn off electrical equipment;
- stop working;
- organize the evacuation of people;
- immediately start extinguishing the fire.
4.4. When electrical equipment catches fire, only carbon dioxide or dry powder fire extinguishers should be used.
4.5. If it is impossible to carry out extinguishing on their own, the orthopedic traumatologist should call the fire brigade by phone 101 or 112 and inform the immediate supervisor (chief doctor, head of the department).
4.6. In case of injury or deterioration of health, the orthopedic traumatologist must stop working, transfer responsibilities to another person, inform the management and seek medical help.
4.7. In the event of injury or deterioration in the health of a subordinate employee, remove him from work and send him to the first-aid post, and, if necessary, call the city ambulance.
4.8. If the integrity of the gloves is broken during medical procedures, they must be replaced immediately, and the hands must be treated with an alcohol-containing skin antiseptic.
4.9. In case of injury with used instruments (including microtrauma), it is necessary to immediately squeeze out the blood, treat the wound with iodine, and take measures for emergency prevention of infectious diseases.
4.10. If an accident occurs, witnessed by an orthopedic traumatologist, he should:
- stop working;
- Immediately report the incident to the immediate supervisor;
- Immediately withdraw or remove the victim from the danger zone;

- call an ambulance or help organize the delivery of the victim to the nearest medical facility.

5. LABOR PROTECTION REQUIREMENTS AT THE END OF WORK

5.1. Accept the results of the work of subordinate employees, control the process of putting their workplaces in order.
5.2. Control the process of storage by subordinates of equipment, devices, materials, medical and disinfectants in specially designated places (inaccessible to patients).
5.3. Clean the workplace, make sure that not currently used devices and equipment are disconnected from the mains.
5.4. Prepare disposable items (syringes, dressings, gloves, masks) for pre-sterilization treatment, and then for disposal.
